We offer Adult Mental Health, Targeted Case Management (TCM) Services
Aralife Case Management Services, Inc offers Adult Mental Health Targeted Case Management to assist clients in gaining access to needed financial and insurance benefits, employment, medical, social, educational, assessment of functional abilities and needs, and other services. These supportive services include working with the client’s natural support system in order to develop and implement the service plan. They also include follow-up to determine the status of the client’s service plan toward enhancing the client’s inclusion in the community.
Case Managers are helpers and service brokers who assess and advocate ensuring that individuals with a severe and persistent mental illness and/or individuals with co-occurring disorders (mental health/ substance abuse) receive the treatment, service and resources appropriate for their day to day needs.
Services
The purpose of case management services is to help patients handle aspects of their lives that might impact whether the patient remains in treatment or has successful treatment outcomes. These services provide assessment, planning, coordination, monitoring, and evaluation of options and resources to meet an individual's specific needs.
